چکیده

Quantitative coronary angiography (QCA) is a validated and widely accepted method to investigate changes in arterial dimension over time. Calibration of measurements is enabled by the use of the coronary catheter as a scaling device. The dimensions and laminar composition of coronary catheters, however, have changed significantly over recent years and the suitability of the current generation of coronary catheters for calibration purposes has not been validated. We therefore recorded 57 coronary guiding catheters on cinefilm, and compared their automated quantitative measurements (Cardiovascular Angiography Analysis System, CAAS) with their true values (precision micrometer). We found an overall underestimation of quantitatively derived dimensions, ranging from -8.9 to +4% for water-filled catheters and from -15.5 to -3.9% for contrast-filled catheters. In conclusion, while the current generation of coronary guiding catheters shows a wide variety in radiological quality, it can be clearly detected by the CAAS system, and is suitable for calibration of QCA measurements (with the exception of the DVI atherectomy catheter), provided that calibration is done on contrast-empty catheters. o 1994 Wiley-Liss, Inc.

Feasibility of virtual 3-Fr percutaneous coronary intervention using standard guiding catheters

INTRODUCTION Recent studies have reported the efficacy of sheathless percutaneous coronary intervention (PCI) using hydrophilic-coated 5-Fr guiding catheters that are one to two Fr sizes smaller in diameter than the corresponding introducer sheath (virtual 3-Fr PCI). However, the limited number of shapes of hydrophilic-coated guiding catheters occasionally makes them difficult to engage and con...

Use of large-caliber coronary guiding catheters for neurointerventional applications.

The use of certain large-caliber (ie, 7F to 9F) coronary guiding catheters for selected neuroendovascular procedures is described in the context of surveying recently developed neurovascular guiding catheters. These large-caliber guiding catheters have proved useful in situations requiring proximal access to tortuous brachiocephalic arteries, permitting coaxial delivery of relatively large neur...

Angioplasty of chronic total coronary occlusions with the use of six French guiding catheters.

The efficacy of 6 French guiding catheters for angioplasty of chronic total coronary occlusions was evaluated in 61 consecutive patients. The duration of the occlusion was determined angiographically, or estimated from an index clinical event. Endpoints were procedural success, defined as recanalization with less than 50% residual stenosis without major complications; and the need to change to ...

Transradial percutaneous coronary intervention for chronic total occlusion of coronary artery disease using sheathless standard guiding catheters

OBJECTIVES Our aim was to evaluate the feasibility and safety of routine transradial approach (TRA) percutaneous coronary intervention (PCI) for chronic total occlusion (CTO) lesions using the sheathless technique with standard guiding catheters. BACKGROUND Transradial approach PCI was applied for CTO lesions. A major limitation of TRA CTO PCI is the inability to use large guiding catheters b...

Successful percutaneous coronary intervention using a 4-in-3 “Slender Mother and Child” PCI technique

We report two cases of successful percutaneous coronary intervention (PCI) using a 4-in-3 "Slender Mother and Child" PCI technique. In both cases, coronary angiography revealed severe stenosis of the right coronary artery (RCA). In both cases, 5-Fr sheathless guiding catheters were inserted into the RCA (virtual 3-Fr PCI) but stents could not pass through because of stents previously implanted ...
